Community Health Worker
Our organization employs Community Health Workers (CHWs) who are integral to the Pathways Community HUB Institute Model. CHWs serve as trusted members of the community, bridging gaps between individuals and essential healthcare and social services.
They offer support, guidance, and advocacy, addressing social determinants of health and improving outcomes for those they serve. Through our partnership with the Pathways Community HUB Institute, we ensure our CHWs receive the training and resources necessary to effectively meet the needs of our community.
Quality Mental Health Specialist
Our Quality Mental Health Specialists provide structured, person-centered mental health support that complements our case management and Community Health Worker services. Through a partnership with Behavior Impact Partners, QMHS services focus on strengthening emotional well-being, coping skills, and stability for individuals and families navigating stress, trauma, and life transitions.
QMHS support includes skill-building, emotional regulation strategies, goal setting, and ongoing therapeutic support designed to help clients function successfully in their daily lives. Services are delivered in a trauma-informed and strengths-based manner, meeting individuals where they are and reinforcing protective factors that promote long-term wellness.
